These two-part carbonless USRSA claim checks/repair tags can help with your customer service and record keeping. Claim checks can be hung from the racquet using the pre-punched hole. One side of the form includes room for the name, phone, address, racquet, string, tension, comments, amount due, date in, and date out. The other side tells the customer the top three benefits to working with a USRSA member stringer. Add $3.75 per 100-pack for shipping and handling.
